Congratulations on being selected as an eligible member of the 2021-22 4.0 Club. 

This year’s 4.0 Club Recognition Program will be on Tuesday, July 19 in the Penn State Fayette Community Center. Registration begins at 5:30 p.m. and the program begins at 6 p.m.

The ceremony will recognize each student, by high school, who has earned a cumulative 4.0 GPA. In order to ensure all recipients are recognized, students MUST complete this form by Thursday, June 30. Please complete the entire form. Once submitted you will receive additional information and reminders about the event. 

You must complete this online form to complete your membership, but only those attending the event will receive the certificate.
